Vaccination Schedule

2 months old

  • Diphtheria, tetanus, pertussis, polio, haemophilus influenzae type b (DTaP-IPV- Hib) (Pediacel 1)
  • Pneumococcal (Prevnar 1)
  • Rotavirus (Rotarix 1)

4 months old

  • Diphtheria, tetanus, pertussis, polio, haemophilus influenzae type b (DTaP-IPV-Hib) (Pediacel 2)
  • Pneumococcal (Prevnar 2)
  • Rotavirus (Rotarix 2)

6 months old

  • Diphtheria, tetanus, pertussis, polio, haemophilus influenzae type b (DTaP-IPV-Hib) (Pediacel 3)

6 months + old

  • Influenza (Inf) every year in the fall

12 months old

  • Meningococcal (Menjugate)
  • MMR #1

15 months old

  • Varicella (Varivax 1)
  • Pneumococcal (Prevnar 3)

18 months old

  • Diphtheria, tetanus, pertussis, polio, haemophilus influenzae type b (DTaP-IPV-Hib) (Pediacel 4)

4 – 6 years old

  • Tetanus, diphtheria, pertussis, polio (Tdap-IPV) (Adacel-Polio)
  • Measles, mumps, rubella, varicella (MMRV)

Grade 7/8

  • Meningococcal (Men-C-ACW/)
  • Human papillomavirus (Gardasil 9) – females
  • Hepatitis B

14-16 years old

  • Tetanus, diphtheria, pertussis (Tdap) (Adacel)
  • Tetanus, diphtheria, pertussis (Tdap) once in adulthood

18 years + old

  • Tetanus, diphtheria (Td) once every 10 years

65 years +

  • Pneumococcal  (Pneumovax 23)
  • Shingles (Zoster) vaccine  (Zostavax)

If your child receives a vaccine through a health care provider, you must report it to the Sudbury & District Health Unit and to our office to update your records.
